ENY Advance Notice of Injury

Process:

• Player gets injured (EF or non-EF player) at our complex
• Coach notifies Division Leader of injury
• Division Leader notifies club leadership and HVSL (Hudson Valley Soccer League) of injury and potential insurance claim
• Division Leader points coach to website URL to share with parent (for EF player) or shares link with coach (for non-EF player)
  • Parent / coach are informed that there is a 30 day window from date of injury to submit claim to HVSL
  • Form is returned from parent to EF Coach or non-EF Coach to Division Leader
• Completed form is sent from Division Leader to Chris at HVSL (soccer@ehysl.org)
